The Consolidated Omnibus Budget Reconciliation Act (COBRA) of 1986 requires most employers to offer extended coverage to associates and dependents who would otherwise lose group health plan coverage. Additional information and an appropriate election notice will be sent if a qualifying event occurs such as:

  • Eligibility for Medicare

  • Divorce or legal separation

  • Death of associate

  • Change in dependency (child)

  • Reduction of hours (change in associate status)

  • Termination by associate or employer (other than gross misconduct)

Under COBRA, you, your spouse or other dependents have 60 days to notify the COBRA administrator if you wish to activate coverage. If you do not notify the plan administrator within 60 days of the qualifying event, coverage is forfeited.
